What is Truncus Arteriosus?  

Tricuspid Atresia is a condition where an infant is born without an important part of the heart called “the tricuspid valve.” This valve plays a vital part in the heart, which is to pump blood between the lungs and body. 

In some newborns, the tricuspid valve is missing or has not properly developed. Tricuspid Atresia usually develops with a Ventricular Septal Defect (hole in the heart). In this condition, blood with and without oxygen mixes and makes it harder for the baby’s heart to pump blood. Truncus Arteriosus is a life-threatening condition and surgery shortly after birth can correct this condition. These children will need more surgeries as they grow.  There are three types of Tricuspid Atresia and the types are based on how large the VSD is. 

1. Large VSD – If the hole in your newborn’s heart is large, then too much blood may go to the lungs. This results in a condition known as Congestive Heart Failure (CHF) 

2. Moderate VSD – If your newborn has a moderate VSD, then the hole is medium-sized. This hole will allow some blood to be pumped to the lungs. Newborns with such a condition will experience less distress than those with large or small holes.  

3. Small VSD – If the hole is small, sufficient blood is not pumped to the lungs to pick up oxygen. A lack of oxygen to vital organs will cause your newborn’s skin to turn blue. This is called Cyanosis.

What are the Symptoms of Truncus Arteriosus? 

The Symptoms of Truncus Arteriosus usually develop in the first few days of life. They include: 

  • Blue coloring of the skin 
  • Poor feeding 
  • Fatigue that occurs during feeding 
  • Excessive sleepiness 
  • Poor weight gain 
  • Shortness of breath 
  • Rapid breathing 

How is Truncus Arteriosus diagnosed? 

Your doctor may hear a murmur when listening to your child’s heart during a physical examination. If your doctor suspects your child may have Truncus Arteriosus, he or she will suggest further tests. 

  • An Echocardiogram (ECHO) diagnoses Truncus Arteriosus. This test will give cardiologists a moving picture of your child’s heart in action.  
  • A Chest X-ray will also be done to check if your child’s lungs have fluid in them. 

How is Tricuspid Atresia Treated?  

If your baby can’t breathe, they will be put on a breathing machine, or ventilator. Your baby will also receive medication to maintain their heart’s function. After your baby’s condition stabilizes, they will need surgery to correct the heart defect. For infants who have Tricuspid Atresia, three surgeries are necessary as the child’s heart and body grows and changes.  

1) Norwood Operation 

In most infants with this condition, the first heart surgery can be done in the first few days of life. This procedure involves placing a shunt in the heart to keep the blood flowing to the lungs. Your baby will need medications and monitoring to detect complications.  A pediatric cardiologist will determine when your baby is ready for the second surgery (Hemi-Fontan Procedure or Glenn Shunt). 

2) Hemi-Fontan or Glenn Shunt 

The second surgery can be done when your baby is between 3 and 6 months old. This operation is called the Glenn Shunt procedure or Hemi-Fontan procedure.   

  • The vein that carries blood into your heart after delivering oxygen throughout your body is called the Superior Vena cava.    
  • The artery which funnels blood from your heart to your lungs is called the Right Pulmonary Artery.    

The Hemi-Fontan Procedure is done to connect the Superior Vena Cava to the Right Pulmonary Artery. 

3) Fontan Procedure  

The third surgery is called the Fontan procedure and can be done between 18 months and 5 years of age. In this surgery, your child’s surgeon will create a path for oxygen-depleted blood which returns to the heart to flow into the arteries that will take it directly to the lungs.
